Registration Form Instructions and Information (for payment by credit card)

PLEASE READ THOROUGHLY BEFORE COMPLETING THE REGISTRATION FORM.

  1. For Registration Date, enter the date on which you are completing the form. This will be considered the effective date of your registration.
     
  2. For Last Name, First Name and M.I., you must use your legal name. No nicknames are allowed. This is especially important if you choose not to supply your Social Security Number (see next item).
     
  3. For Student #, you may supply your Social Security Number (SSN). If you prefer to not use your SSN for this purpose, leave this field blank.
     
  4. For Legal Name Verification, you must check the box verifying that you have supplied your legal name.
     
  5. For Course Selection, select the course appropriate to the type of credit you are pursuing.

    It should be noted that students pursuing 3 Undergraduate College credits are required to complete additional coursework beyond those pursuing 60 Professional Development credits, and students pursuing either 120 Professional Development credits or 3 Graduate College credits plus 67.5 Professional Development credits are required to complete additional coursework beyond those pursuing the Undergraduate credits.

    Additionally, please note that if you plan to take the course for 3 Graduate College credits plus 67.5 Professional Development credits, the all-inclusive cost 
is $799.00 (covers Words That Cook Tuition, Materials & Fees and Fitchburg State College Registration & Administration Fees in full).

    If you plan to take the course for 120 Professional Development credits, the all-inclusive cost is $699.00 (covers Words That Cook Tuition, Materials & Fees in full.)

    If you plan to take the course for 3 Undergraduate College credits, the all-inclusive cost is $699.00 (covers Words That Cook Tuition, Materials & Fees and Fitchburg State College Registration & Administration Fees in full).

    If you plan to take the course for 60 Professional Development credits, the all-inclusive cost is $529.00 (covers Words That Cook Tuition, Materials & Fees in full and Massachusetts Association for the Education of Young Children Certificate fee, where applicable).
     
  6. For Course Start Date, enter the first day of the month in which you plan to begin your coursework and the corresponding calendar year (e.g. July 1, 2006).

Once we have verified your registration, course materials will be delivered. Registering by the 20th of any month allows sufficient time for course materials to be delivered for an upcoming course start date.
